Requirements for late registration of birth certificates bared
The city’s Local Civil Registry Office (LCRO) urged individuals whose birth certificates have not yet been registered to avail of the late registration of their documents to prevent delays in their future transactions with government and private entities needing the said important document.
LCRO Registration Officer IV Marieta Joven said that it is important that individuals with unregistered birth certificates are able to produce the mandatory and other related requirements to support their late registration.
Among the mandatory requirements for late registration of birth certificates are negative certification from the Philippine Statistics Authority (PSA), community tax certificate or valid identification card, 2×2 unedited picture with white background taken within three months from date of registration, national ID and barangay certification as proof of residency.
Further, the applicable mandatory requirements are marriage certificate of applicant, marriage certificate of parents, death certificate of document owner if necessary, death certificate of parents if necessary, birth certificate of siblings, affidavit or a sworn statement stating the present whereabouts of the mother and the reason for her inability to personally appear, valid passport or Bureau of Immigration (BI) clearance certificate or alien certificate of registration identity card if one of the parents is a foreigner, affidavit of delayed registration executed by the applicant with corroborating statement for out of town applicants, affidavit of two dis-interested persons, affidavit of acknowledgement for children born before August 3, 1988, affidavit of admission of paternity and affidavit to use the surname of the father.
The applicants are also required to produce any two of the documents such as baptismal certificate, school records, yellow card or baby book, voter’s registration form, Philippine Health Insurance Corporation membership data registration, medical records or pre-natal record of mother and passport showing the name of the child, date and place of birth, name of parents among other important details needed for the said purpose.
Joven said that the requirements are important to fully support the desire of the applicants to have their birth certificates be subjected to late registration for smooth transaction with concerned government and private entities. – Dexter A. See
